Dear CDFellow,
TDR would like to harmonise the progress report for the CDF. The objective is to use the same form for all the grantee that could be posted on the website.
This is the draft of the first version of the forms. Could you , please we need inputs from all CDFs? Dead line of End of April will be fantastic.

Dear Pascal,
I hope we kept to your deadline baring the Labour Day celebration.This is the summary of suggestions from CDFellows on the First draft a first version of the CDF reporting form. The fellows are in consensus that this template report is straightforward and content should be ok. But suggest however that:
(1) The overall appraisal should be from the fellow's perspective, with a place for overall comment and endorsement by his/her supervisor/ line manager in the host institution
(2) There should be a way to foster joint evaluation of the progress between the supervisor and fellow before report writing. That is a general comments section from the industry supervisor as well on general progress of the fellow.
(3) Maintain the fellow's report separate and independent from that of the host industry supervisor. This allows the fellow to be frank in his/her assessment. It also allows for the objective assesment of both the fellow and the supervisor. A joint report with the supervisor could hamper sincerityOther desirable suggestions include:
(i) Impromptu reporting method or mechanism to rectify issues in case the fellow is having challenges: a mechanism that does not have to wait for the 6/12 month report.
(ii) Sharing these reports with past CDFellows to help the concerned fellow make the best of their attachment.
(iii) Beginning this reporting document with the acknowledgments, then the list of abbreviations and also insert gender (female/male) -
And it will be useful early on to discuss the format of the report with your supervisor. This is once the final format is out. It will make life easier 6 months down the line to do the report with your supervisor.

You raised a good point Dr Julius
Regardless of the reporting mechanisms I think there should be common understanding between fellow and supervisor/industry on the needs and challenges of the fellow. Sharing of information is therefore essential.
I agree that a fellow should be frank and independent in his/her assessment and I think part of this frankness is to openly share his or her views with supervisor/industry.
Despite difficulties in some places a fellow and supervisor should discuss their views on the progress somehow. I am not sure if this should be before or after the independent reporting by each other and whether there could be negative impact on the relationship between the two if some contentious aspects of the report are not discussed before. At the end of the day wisdom may be crucial in dealing with difficult supervisor.
